We ask that you fill out another application, bring in a current utility bill, with your physical address on it, and we need a photo copy of your Louisiana Drivers License or Louisiana Picture ID. Our Mission:To provide recreational facilities and activities for the citizens of Ward Four, District One. Our goal is to better enhance the lives and health of our local members. All District One citizens are urged to participate in this endeavor.
Eligibility:Citizens that reside in Calcasieu Parish, Ward Four, District One. Proof of residence must be provided to enroll. See "Eligibility"
